A system change was applied incorrectly, which caused form submissions, saves and requests to fail for approximately 1 hour between 5.15pm and 6.15pm NZDT. Form completion itself was unaffected, and no data was lost: every affected submission was held safely and has since been restored to the state the user intended. Affected customers have been contacted directly with the details of the documents involved. We are adding additional automated checks to ensure this doesn't happen again.
